Oregon Statewide Assessment System (OSAS) Annual Notice

In 2010, Oregon adopted higher K-12 standards in English Language Arts and Math to ensure all students move from grade to grade with the academic knowledge and skills necessary for success beyond high school. The state tests: challenge your child to think critically, go beyond multiple choice and ask your child to explain their answers, act as a snapshot of your child’s progress, and help us identify schools that need additional supports to ensure more students are meeting higher standards.

Statewide tests provide educators and administrators with information about what educational approaches help students show progress. Test results may be considered to determine where additional resources are needed. Your child’s participation is important to ensure schools and districts receive the targeted information they need to help all students succeed. Opt-Out Information for the Oregon Statewide Assessment System (OSAS) in English Language Arts and Math

To opt-out of the Math or ELA assessment, please complete the form below and submit it directly to your child's school. To best support school district planning, you are encouraged to submit this form before testing begins. This form is only valid for the 2024-25 school year. Parents or adult students are required to submit an opt-out form annually should they wish to be exempted from statewide summative tests.

Exemption Information for OSAS ELPA Summative

Under Oregon Administrative Rule 581-022-1910, parents may request that their student be exempted from taking the Oregon State Assessment System (OSAS) English Language Proficiency Assessment (ELPA) to accommodate a student's disability or religious beliefs. Please complete the form below and submit it directly to your child's school. To best support school district planning, you are encouraged to submit this form before testing begins.

Annual Notice and Declining Participation Form for the SEED Survey

Oregon law (House Bill 2656) permits parents or guardians to decline, either verbally or in writing, to have their student participate in the SEED Survey. To decline participation in the SEED Survey, parents or adult students may submit the SEED Survey Decline Participation Form directly to the school.
